Educated at Oundle, Cambridge and Guy's Hospital, Clarke served throughout the Second World War as a medical specialist in the Royal Naval Volunteer Reserve. Following the war he became a medical registrar in Birmingham, and later a consultant at the David Lewis Northern Hospital in Liverpool. A growing interest in research led him to a career change, and he became a Professor of Medicine at Liverpool University.
